Launched in 2018, VNA participated in the only Medicare-sponsored home-based palliative care program in North Texas (Medicare Care Choices Model). The VNA Care Choices program provided palliative care, care coordination and supportive services in the home without discontinuing other treatment options. The CMS pilot program ended July 1, 2021. It is our hope that this pilot will become a permanent benefit in the near future as data shows that patients’ outcomes greatly increased while costs were lowered.
