Communication is key. At VNA, we know hospice is uncharted territory for many of our patients and families. Our goal is to make your experience as smooth and transparent as possible. One of the best ways to help ease your mind about hospice care is to communicate your fears, questions, and concerns with your hospice team.
Hospice is a unique form of health care that is traditionally community based and provided by an interdisciplinary team of doctors, nurse practitioners, RN case managers, chaplains, medical social workers, volunteers and others such as a music therapist. With this extensive interdisciplinary team, you will likely communicate with multiple team members throughout your hospice journey.
Questions are always welcomed. It is important for you to be informed and updated regarding the many aspects of the hospice process.Our care team is always available to answer questions about treatments or the care plan and VNA offers a 24/7 nurse line for patients and their families experiencing an emergency.
Following admission to hospice, a care plan is established outlining what care a patient needs and will receive. Patient and family input is critical during this process to determine the best plan for each individual patient.
You know your situation better than anyone. If you or your loved one has been experiencing abnormal symptoms, are experiencing pain, or changes in behavior, it is vital to share those developments with your care team. Every piece of information—no matter how small it may seem—can help your care team better understand you, your needs, and which treatments should be considered.
The frequency of visits is also highly personalized and determined by the care team in conjunction with the patient and family and b to best support and meet the needs of each patient and his/her family.
You have the right, by law, to choose your hospice provider and if you choose VNA, we promise to keep an open channel of communication with you from the very start.VNA provides the quality of care you know you can trust.
